About the role

The Care Coordinator is responsible for providing advanced clinical support, leadership and coordination of clinical care for residents with complex needs.  You will provide mentorship and guidance for a care team of PCW's and Registered Nurses within our facility.

In this role you will:

  • Oversee and co-ordinate the completion of focus assessments of individual resident needs and record findings within a Positive Wellbeing Model;
  • Ensure that care plans are developed and reviewed based on the assessment findings of individual resident needs;
  • Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;
  • Monitor that resident documentation is accurately reflective of care provisions and is in accordance with organisational requirements;
  • Co-ordinate the care provision of external service providers;
  • Assist in the review and maintenance of clinical programs and initiatives;
  • Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines.

In this position you will be working on the floor Monday – Friday, PM shifts.

As the Care Coordinator, ideally your experience and qualifications include:

  • Current AHPRA registration as a Registered Nurse, with min 2 years of aged care experience;
  • Experience in managing and leading teams in the aged care sector;
  • Strong knowledge of the aged care accreditation system and compliance;
  • Demonstrated knowledge of AN-ACC;
  • Experience in educating, mentoring and motivating staff;
  • Superior communication skills both written and verbal;
  • Be empathic, compassionate and committed to Resident Centric Model of Care.
