About our Home Care Nurses

As our newly appointed and committed Nurse, your days will be filled with variety and purpose. Situated in Hervey Bay, your responsibilities will span across the Wide Bay region. Your role will involve delivering clinical care to clients residing within their homes throughout these areas.

 

There is flexibility with the shift scheduling. 

You will be required to

  • Provide advanced clinical care to clients including chronic disease management, medication management, palliative care, catheter management, stoma care, wound management, and diabetes management;
  • Assess, plan, implement and evaluate nursing care for home-based clients with complex needs and record findings within a Positive Wellbeing Model;
  • Ensure that care plans are developed and reviewed based on the assessment findings of individual client’s needs, in accordance with the organisation's timelines;
  • Monitor and ensure that client documentation is accurately reflective of care provisions and is in accordance with organisational requirements;
  • Providing timely and appropriate health care and education to clients and carers;
  • Coordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines;
  • Provide guidance to Home Care staff;
  • Work collaboratively with allied health professionals and other clinical professionals;
  • Support and consult with clients, their families or representatives.

 

About you

You have the ability to prioritise and manage workloads according to client needs and are able to work independently or as part of a multi-disciplinary team. You can effectively communicate at all levels enabling you to achieve best nursing practices in accordance with legislation and quality improvement.

 

Essential experience and qualifications will include:

  • Registration with AHPRA as a Nurse with a current Practicing Certificate;
  • Post-graduate experience as a Nurse;
  • Strong understanding of the principles of primary health care including providing services in a consumer directed framework;
  • Possession of First Aid and Cardiopulmonary Resuscitation (CPR) certificates;
  • QLD driver’s license with access to a reliable and comprehensively insured vehicle (an allowance will be provided for use of private vehicle);
  • Experience within the Home Care sector is highly desirable;
  • High level computer skills;
  • Possess or willingness to obtain a National Police Check and a National Personal Insolvency Check.
  • Possess excellent communication and interpersonal skills, have the ability to work well in a fast-paced and dynamic environment, and have a compassionate and empathetic approach to patient care.
