About the role

Churches of Christ are currently looking for Care Partner within our dynamic Home Care Hervey Bay, as a Care Partner you work across all Support at Home pathways, including Restorative Care, End of life and Assistive Technology & Home Modifications, contributing to the mission and strategic goals of Churches of Christ as part of the Seniors Living team.

They provide flexible care planning, education and advocacy to enhance participant safety, health, wellbeing, and quality of life, incorporating wellness, reablement and culturally safe practices.  Where clinical needs arise, they partner with qualified clinicians for complex care planning.

As a Care Partner, you will:

  • Develop individualised and culturally appropriate care plans aligned with assessments, goals, preferences, risks and AT HM needs; ensure care plans identify wellness and reablement approaches to meeting participant goals
  • Collaborate with relevant team members to support the coordination of service under the End of Life and AT HM Pathway (within scope), ensuring alignment with participant goals and safe integration of supports
  • Provide essential direct care services to participants as required, in response to individual care needs and service demand (such as personal care, medication support and transport).
  • Deliver monthly care management interactions to all participants

 Who we're looking for

You're a natural people leader with a heart for community care and a talent for getting the best with your team. You're confident navigating the aged care space and thrive in a fast-paced, mobile work environment. 

Our ideal candidate

  • Certificate 3 in Home and Community or Individual Support
  • One years' experience in home care and planning, co-ordinating and implementing care services in the home care environment
  • HLTHPS006 - Assist clients with medication
  • HLTAAP001 - Recognise healthy body systems
  • CHCCCS011 - Meet personal support needs
  • Currently holds, or willing to obtain a First Aid and CPR qualification
  • Currently holds, or willing to obtain a current Police Check
  • A current driver's license and willingness to use your own comprehensively insured vehicle for work purposes. (Reimbursement of $0.96 per KM is provided for use of own vehicle)
  • Hold valid Australian Work Rights - Unfortunately we are not currently able to sponsor
