Are you an experienced Clinical Coordinator or senior Registered Nurse ready to progress into a Care Manager role? Perhaps you already have Care Manager experience and are looking for a supportive service where you can make a meaningful impact.

Churches of Christ is seeking a clinically capable and people-focused Registered Nurse to join Moonah Park Aged Care as Care Manager.

You don’t need to have held the Care Manager title previously. We are open to an emerging leader with strong residential aged care experience, sound clinical judgement and the confidence to support and guide others. You’ll work closely with an experienced Service Manager and broader support teams as you build your leadership capability.

Located in Brisbane’s inner north, Moonah Park offers a convenient commute and is within walking distance of both Oxford Park and Mitchelton train stations.

About the Role

As Care Manager, you will work alongside the Service Manager to provide clinical leadership across the service, supporting Registered Nurses and care teams to deliver safe, high-quality and person-centred care.

Your responsibilities will include:

  • Providing day-to-day leadership, guidance and support to clinical and care teams;
  • Coordinating resident assessments, care planning and clinical reviews;
  • Monitoring clinical practice, documentation and resident outcomes;
  • Supporting the accurate completion and review of AN-ACC documentation;
  • Building collaborative relationships with residents, families, allied health professionals and external providers;
  • Supporting compliance with the Aged Care Quality Standards, clinical governance requirements and reportable incident obligations;
  • Identifying opportunities to strengthen clinical practice and improve care delivery; and
  • Promoting a positive culture of safety, accountability and continuous improvement.
  • Pay Rate: $60 - $63 per hour

About You

You are a capable Registered Nurse who combines sound clinical judgement with warmth, accountability and strong communication skills.

You may already be working as a Care Manager, or you may be an experienced Clinical Coordinator, Clinical Nurse, senior RN or team leader looking for the opportunity to step into broader clinical leadership.

To be successful, you will bring:

  • Current AHPRA registration as a Registered Nurse;
  • Demonstrated experience within residential aged care or a relevant clinical environment;
  • Sound knowledge of contemporary clinical practice, care planning and documentation;
  • Experience supporting, mentoring or coordinating clinical and care staff;
  • Strong communication, problem-solving and decision-making skills;
  • The ability to build trust with residents, families and colleagues;
  • Current CPR and First Aid certification, or willingness to obtain;
  • A National Police Check, or willingness to obtain; and
  • Current Australian working rights, as sponsorship is unavailable for this role.

Previous Care Manager experience is welcomed but not essential. Postgraduate qualifications in gerontology, dementia or aged care will also be highly regarded.
