Lifestyle Coordinator

At Gracehaven Aged Care, we are seeking a warm, organised, and motivated Lifestyle Therapy Team Leader to lead engaging lifestyle programs that support residents to live fulfilling and connected lives within our aged care community.

About the Role
In this rewarding role, you will lead and support the Lifestyle Therapy team to deliver meaningful activities that promote wellbeing, independence, and social connection for residents.

You will work closely with residents, families, volunteers, and the wider care team to create a welcoming environment where residents feel valued, included, and encouraged to participate in activities that reflect their interests and preferences.

Key Responsibilities

  • Lead and support Lifestyle Therapy staff and volunteers
  • Coordinate engaging group and individual lifestyle activities
  • Develop and review person-centred lifestyle programs
  • Encourage resident participation, wellbeing, and connection
  • Build positive relationships with residents, families, and team members
  • Maintain accurate documentation and activity records
  • Contribute to a positive and supportive team culture

About You
You are passionate about improving the lives of older people and enjoy creating meaningful experiences for others. You bring strong leadership skills, a caring approach, and the ability to build genuine relationships with residents and colleagues alike.

To be successful, you will have:

  • Certificate IV in Leisure and Health
  • Experience delivering lifestyle or recreational programs in aged care
  • Previous leadership or team coordination experience
  • Strong communication and interpersonal skills
  • Current First Aid and CPR Certificate, or willingness to obtain
  • National Police Check, or ability to obtain
