About the Role

The Transition to Adulthood (T2A) team at Churches of Christ supports young people as they move from out of home care into independent adulthood. Funded by the Department of Child Safety, Seniors and Disability Services (DCSSDS) through the Extended Post Care Support and Next Step Plus initiatives, our programs are evidence informed, structured, and tailored to the unique needs of each young person.

Our practitioners work alongside young people and key stakeholders to build essential life skills, strengthen support networks, and create pathways toward long term stability. We are seeking someone with initiative, compassion, and the ability to work both collaboratively and autonomously.

Reporting to our Team Leader, you will draw on your professional experience to deliver high‑quality, youth‑centred support. You will use a blend of assessment and intervention skills, crisis response, and therapeutic approaches to empower young people as they navigate the transition from care to independence.

Young people leaving care often face complex challenges, navigating systems, building safe support networks, securing stable housing, accessing mental and allied health services, and engaging with education or employment pathways. Your role is to walk alongside them, offering practical guidance and emotional support while building their confidence and capability. Our overarching aim is to set young people up for long‑term success without creating reliance on paid services.

About You

You are passionate about walking alongside young people as they transition to adulthood, helping them build confidence, capability, and community connections. With a background in care coordination or case management, you understand the profound impact a supportive mentor can have. You bring a values driven approach, a learning mindset, and the ability to build strong, trusting relationships.

You will:

  • Provide coaching and advocacy for young people transitioning from care.
  • Collaborate with stakeholders including natural mentors, Child Safety, health providers, and community services.
  • Mentor young people to identify goals, plan pathways, and creatively overcome barriers.
  • Offer practical support across housing, employment, health (including mental health), and wellbeing.
  • Work respectfully with Aboriginal and Torres Strait Islander young people, families, and communities to ensure culturally safe and inclusive practice.

What you will bring to our team:

To be successful, you will have:

  • Degree qualification in social work, psychology, social sciences or a related discipline (or an equivalent level of expertise (more than 4 years’ experience) attained through previous appointments, service and/or study. Services)
  • Applied knowledge and understanding of child protection and out-of-home care frameworks, including knowledge of, or the ability to acquire knowledge of the Child Protection Act 1999.
  • Demonstrated ability to complete assessments for care.
  • Demonstrated knowledge of the contemporary challenges for Aboriginal and Torres Strait Islander peoples.
  • Current driver’s licence – P2 minimum.
  • Demonstrated understanding of the Domestic and Family Violence Protection Act 2012 and Child Protection Act 1999.
  • Sound understanding of child development and trauma theory to inform assessment and decision making.

Mandatory prerequisites

  • National Police Certificate (or ability to obtain).
  • Working with Children Check (Blue Card) (or ability to obtain).
  • Child Safety and Personal History Screening Check (LCS), where applicable.
  • Current driver’s licence.
