Care Manager

Are you an experienced Registered Nurse ready to step into a key clinical leadership role?

We are seeking a Care Manager to lead the delivery of safe, high-quality and person-centred care at our Palms Aged Care service in Ingham. Working closely with the Service Manager, you will provide clinical leadership, support and guidance to our nursing and care teams while strengthening quality, compliance and resident outcomes.

About the role

As Care Manager, you will:

  • Lead, support and develop Registered Nurses, care staff, allied health professionals and volunteers.
  • Oversee resident assessments, care planning and clinical documentation.
  • Monitor clinical practice, care outcomes and quality indicators.
  • Coordinate care delivered by internal teams and external healthcare providers.
  • Review AN-ACC information to support accurate and appropriate submissions.
  • Ensure clinical programs, policies and procedures meet legislative, regulatory and professional requirements.
  • Support incident management, risk mitigation and continuous improvement activities.
  • Build positive relationships with residents, families, employees and healthcare professionals.
  • Promote a collaborative, accountable and resident-focused team culture.

This position is designated as a Responsible Person under the Aged Care Act 2024 and plays an important role in supporting compliance with the Aged Care Quality Standards, Code of Conduct and reportable incident requirements.

About you

You are an experienced and approachable clinical leader who can balance strong governance with compassionate, person-centred care. You will bring:

  • Current registration with AHPRA as a Registered Nurse.
  • A minimum of five years’ post-graduate experience in aged care or an acute clinical setting.
  • Demonstrated clinical leadership and people management experience.
  • Strong knowledge of contemporary aged care practice, clinical governance and quality standards.
  • Well-developed communication, decision-making and problem-solving skills.
  • Confidence managing change, clinical risk and continuous improvement.
  • Current CPR certification and an unrestricted driver licence.
  • A current National Police Certificate or the ability to obtain one.
  • Current Australian working rights, as sponsorship is not available for this role.

A postgraduate qualification in gerontology, dementia care or another aged care-related discipline will be highly regarded.
